I think this is an Ipad issue, but Yes, my bad for not specifying:
iMac mid-2006 running OS/X 10.6.8 and the latest version of iTunes.
iPhone 4S running iOS 5 (latest)
iPad 2 running iOS 5 (latest)
I use MobileMe for all the syncing you mention and have for years. I can't use
iCloud because the iMac with 10.6.8 won't handle it (requires Lion). I sync
both via USB directly with the iMac and also via wifi.

The music itself ("songs") are copied just fine. It's the playlists thar
organize the songs that don't get copied.

I do not manually mange my music. I check to make sure that box is not checked
often, and it never is (and is not right now)
Not sure what else to try before whining to Apple support....
