With this change `SingleMapWriter` is now the same same as
`NullableMapWriter` so they can also be combined and `SingleMapWriter` can be
removed. This is also the same for `SingleMapReaderImpl`.
@siddharthteotia and @jacques-n do you have any objections to removing the
`SingleMapWriter/Reader` classes here as well?
