This is, as the name hints, a fairly major fix. I've re-written the rule
application logic in a functional/oo style, rather than following the PHP
string-as-data-and-state approach.
All tests now pass. All performance tests now run in sane time. The
intermediate output from these is more sensible than before.
It's possible that we are producing encodings that don't exactly match what the
original PHP produces. However, the encodings I've looked at by eye look good
enough to use.
