### Review finding: app-server startup can exceed the judge deadline `CodexAppServerJudge._connection` waits indefinitely for the shared connection lock, and `_start` gives each initialization RPC the original timeout. Contention or multiple slow startup stages can therefore exceed the remaining judge deadline before `turn/start`.
