I had the same problems and they all went away since I started to use the newest firmware version (0x02050032, pvr_1.18.21.22301_inf.zip). >From then on, I *never* have had any problems with my card (using the Linux-2.6.9 kernel and ivtv 0.2.0rc3a-0.2.0rc3c drivers).
