Make sure you shut the system completely off for about 30 seconds, then
restarted.  Also try cat /dev/video0 > /dev/video16 and see how that
works for you too.  This sounds like something that happens if the
card was already in a bad state then the chip needs a full reset from
a cold boot (it holds settings very very well).  Also what version last
worked, and what happened in between till things didn't work.
