There's got to be something in your config causing this because by 
default Gentoo will run "modprobe" on each MODALIAS that udev finds. 
It's possible udev isn't finding everything it needs to. If you're on 
~arch (and as such running baselayout-2.0.0) you can insert:

modules_2_6="ivtv" into /etc/conf.d/modules and it should force a 
regular modprobe ivtv for you which appeared to work for you.

That being said, Gentoo does not filter any modules out by default and 
leaves that entirely up to the user to configure.
