I don't know if this will work in JAWS 4.5, but you can give it a try. Press Windows key+M to minimize all applications, and focus JAWS on the desktop.
Press Tab until you hear JAWS announce Start, or something like that. Now press Enter to open the start menu. Most of the time my start menu won't open using the windows key. It just says "start button; press enter to open" but will not open anything. I know that programs is the second P on this menu but pressing P twice and entering does nothing.
