In Word 2007 it's alt-t, o, then control-tab to 'advanced.

When I press alt+control+i Jaws announces insert mode active. When
I press those keys again it still says insert mode active and type
over is not activated. Am I doing something wrong?

To toggle between typeover and insert modes, press alt-control-i.
