Or possibly the Alt key is not working properly.  Even though you are
holding it down, maybe it really isn't staying down, or maybe you are
shifting your finger prior to hitting the Tab key.

It sounds as your tab key isn't working. Or you aren't hitting the
combination correctly. Try holding the alt key and than tap the tab key.
