I don't have the file any more, it died with my old computer. Is there a way I could make a new file and set form fields into it? Thanks.

I will be curious if anyone else can identify a mechanism that one could use that would allow text entry that should appear on a single physical line that does not move the text that follows it that does not employ either a table or form. It's been ages since I used Word 2003, so perhaps there was some method that was discontinued in later versions of Word or else that I am completely unaware of.
