Sorry for my late reply,

1. I do not really understand your question. I only draw the silk lines 
and place pads. I have not discovered a way to do courtyards, keep-outs etc.

2.  The only way I found is to make the copper pad as large as the hole 
(if you make it smaller you get an error). This is not very elegant but 
a common practice in many ecads.
