I think you are on the right track with XML-FO.

This looks like it might be just the thing to turn our XML+CSS into
XML-FO. But now we need an XML-FO to pdf or postscript processor. Does
an open source implementation exist? Apparently my colleagues have been
down this road before but could not find an open source implementation.
