I got one from WalMart. IIRC, it was around $20. I haven't had any problems. I don't see a name on it. It's 120V AC, 60Hz, 4.5W with the power cord running right into the unit (no wall wart), catalog #PH61155. The UL numbers on it are *truly* tiny but appear to be 40J6 E84533. "Made in China". It has 5 RCA jacks. One just says "Video". The other four are Left and Right channel, Audio In and Audio Out. It has two RFs, one "From Ant", the other "To TV". And finally, it has a switch to select channel 3 or 4. It's roughly comparable in size to a romance novel. Oh yeah, on the front it has a red LED to indicate channel 3 and a green one for channel 4.

But before you spend money on a new one, are you sure the problem is in the modulator? You could have bad connections inside the TV. And FYI, TVs *have* come down a *lot* in price since such ancient models were sold. ;-)
