I have an inflammatory form of arthritis (it's not rheumatiod arthritis, but similar and is treated with many of the same drugs) and I use a variety of pillows and a variety of bobbins. I generally do Torchon, some Tonder and a bit of Bucks. I usually have at least 3 or 4 pillows going at one time. What I have found is that switching off from one pillow and working technique to another is the biggest single factor in not aggravating my hands. I'll work on a flat pillow with Midlands bobbins in a "palms down" manner for a while (maybe an hour or so), then switch to a bolster and continentals in a "palms up" position. I do notice that when my hands are *really* bad, the "palms up" with continentals is easier. All in all, though, bobbin lace in any technique is easier than most other textile crafts- knitting is all but impossible except in short bursts on my best days.
