I have rheumatoid arthritis, and I've got both continental (Bruges) bobbins
and Midlands. I don't find the Bruges ones that easy to use because, even
with a flat pillow (I have it at an angle) the continental bobbins roll and
the threads sometimes unravelled and fray. I have the pillow at an angle so
that the front edge is lower than I would comfortably work at it if it was
level. By having my wrists low (almost in my lap), I find there's less
strain on them moving them over the pillow than if I hold my forearms
parallelish with the ground, as I would for a comfortable pillow height if
it was flat. So I usually use spangled bobbins.

When my hands aren't good, I lift spangled bobbins up to move them by
putting my forefinger into the centre of the spangle, thumb on the outside,
so I'm holding the bobbin by the large bottom bead between my thumb and
forefinger. It's slow but comfortable.
