I distinguish by account name, but I keep it very short and I also
append the last four digits of the card.  In my case, I have two
different credit cards from the same bank (i.e., a Visa and
MasterCard) and correspondence & bills always say "your account ending
in XXXX".  So I have Liabilities:CC:BoA1234 and also
Liabilities:CC:BoA5678.

I tend to just say "Transfer" since the exact account details are
shown below.  I don't use transfers very often, though.  I do use a
particular Code such as "EFT" or similar.
