I have organized my ledger journal files by month, so that all transactions 
for a month are in the same file.  I have a template ledger file that 
contains transactions that are recurring each month, with an XX for the 
month, and a typical or estimated value for the transactions.  When I need 
to add a transaction for the next month (most often scheduling a credit 
card payment to happen on the due date that is in the next month), I copy 
my template file to the next monthly file, edit the dates on all 
transactions to be correct for the new month, then adjust the dates on the 
few bi-weekly transactions to be correct.  

For transactions where I need to take special action for the bill to be 
paid, I leave the transaction marked with a ! to indicate it as pending 
until I actually make the payment.  This lets me see what I need to do for 
the next month or so with the --pending flag on the register report.  For 
automatic transactions that I don't need to do anything for them to happen, 
they are entered in the template file as uncleared.  

This approach has worked pretty well for me for the 6 months or so that 
I've been using ledger.
