It is totally up to you. If you choose to have AKA names show in the Index and Names List, then you can always find the person. I have many mediaeval ancestors who are known by many different names, titles, and spellings. I enter a Duke, for instance, in many different ways: 1. Last name - whatever I first found 2. AKA last name - the dukedom as the last name with the others as long given names 3. AKA last name - another way I have found it spelled (and there are up to 12 on some names!) And so forth.
I often change my MAIN last name, depending on what the majority of sources use, but since I have all the last names listed, it really doesn’t matter. I can always find the person. And yes, when several generations have the same names and titles and alternate spellings, death and birth dates are helpful.
