There are lots of ways to do this. I use an actual blank _____. It works for me, although I'm sure there are reasonable arguments against it.
The only thing I would absolutely avoid is using an abbreviation like LNU (last name unknown). I've seen many people later confuse such abbreviations with actual surnames.
