Try it Sherry, and I think you will find that a place for address, phone number and email address is not shown on a blank Family Group report, despite the option being selected. It will show on an individual blank report however, under the name of the individual.

PAF 5 includes a box at the bottom of the first page of blank reports where
those details can be filled in (by a relative). It is similar to the normal
box which is completed with the preparer's details for normal reports. Roots
Magic provides a box but no defined areas to complete only for a Family
Group record. It does not have an option for an individual report.

Such an option - "Prepared By" - would be a good option to have on all
reports - blank or completed.
