OK, I was smoking crack when I first wrote this.  There is no clockwise node
numbering in the pyramid!!  Momentarily, I will check in changes that allow
build_cube() to take PYRAMID5 as the element type.   Similarly to the tet
case, we initially build a hex mesh and then split each hex into 6 pyramids.
