I think it would be more reasonable if Y-extent would override skylines when set. However, this does not help with \with-dimensions as far as I can see. In the case of \with-dimensions, we need to get the overriding information attached to the stencil in a manner that is able to override the stencil's contribution to a skyline. Since we also need extents for alignment purposes, we probably still need to have them stored in a stencil, but we'll likely also need to record whether the extents have been stored explicitly or are just a consequence of the skyline calculation.

At first glance, this would appear to make things like an Y-extent override somewhat easier to implement than \with-dimensions.
