My preferences are with 1 and 3 and those for different scenarios (see below).
IMO 2 looses optically against 3.
4 and 5 IMO clutter the music.

I would use 3 in situations, where the singer essentially know the language.

I regularly use something similar to 1 when singing in a language I have no 
understanding whatsoever, aka when I'm just singing arbitrary sounds (from my 
perspective).

In those situations I find it helpful to have it everywhere and don't care 
about repetitions. Whatever helps during performance is good.

In situations where I would prefer 1 I sometimes create my own version of the 
music with my personal understanding of the pronunciation replacing the 
original lyrics or put the original lyrics in italics as a 2. stanza below. I 
find that extremely helpful during performance.

As always others will have other ideas.
