We have been digging down and there might be an issue with the IOCDS and
that we have not done a POR on the box for awhile (only dynam activates).
this was found out by checking the subchannel and chpids file in /proc .

Let us carry on digging and when we resolve I will post a breakdown.

Sure, but yast is on the initrd.  Look at the fourth line put out by the
welcome message: "Then start installation with the 'yast' command."

The fact that you're doing an insmod command for dasd_mod, then both
dasd_eckd_mod and dasd_fba_mod, but nothing is showing up in
/proc/dasd/devices is disturbing.  Do you get any "registration" messages on
the system console (real or virtual) when you do that?  You should see
_something_ if the devices are really attached to the system.  As Leland
suggested, you need to make sure that the system has access to those device
numbers.
