I have seen this message before also.  In most cases, I had defined the
addresses without the 0x prefix or somehow wrong.

Another situation where I saw this message is on a mult port OSA card where
you had to specify the adapter number (not always 0).  So, if your OSA card
has more than one adapter, you have to specify 1 or 2.
