SLES9 SP3 does not require you to do a format of the dasd devices before YaST2 
will see them... so this is out of the Question.
Your modules are not loading correctly, you need to make sure that your initrd 
and linux kernel file are both from the SLES9 SP3 CD1 and that your install 
source is pulling those.

Also, You only use the dasd=  parameter when your doing a zVM install not an 
LPAR install.
