I forgot to say that the most common reason for this error is a failure to
correctly answer all three of these questions:
1.  Which card is this chpid on and where is it in the cage?
2.  Which port goes to which chpid? (for multiport cards)
3.  Which port is (e.g.) "2/7" on the switch?

So usually the wrong OSA port is plugged into the right switch port, or
the right OSA into the wrong switch port.  Sometimes both.  Obviously only
one out of the four possible combinations is the correct one.  :-)

The second most common reason is a bad cable.
