If you are using sendmail, you'd use the access.db by
1. Enabling support for it in sendmail.cf (using the .mc file) - most stock
distro configs have this enabled already.
2. Adding an entry like
gtcdrom.com 550 Get lost - no spam allowed here
running makemap on it (makemap hash /etc/access.db < /etc//access).
Depending on your unix, it might be dbm or some other db format - and if you
are using a 8.10.x or newer sendmail (you are using 8.9.3 right now) the
paths have changed to /etc/mail
See http://www.sendmail.org/faq/ (and http://www.sendmail.org/antispam.html)
for this and much more. There's also a paper on brettglass.com which details
some fairly efficient anti-spam rulesets in sendmail. Don't have the URL
right now, but "brett glass spam sendmail" will turn it up immediately on
google :)
