Basically, nothing. That is, if you consider email client features alone.
Mutt cannot brew your coffee, though. :-)

I used VM (another emacs mail client) for about a year, and then shifted
back to mutt (a brief stint with kmail in between). Once you are used
to mutt, its quite hard to change, because of the incredible amount of
control it offers you over your email.
