Jozef -

We've seen this many times.  It is related to high humidity and dirty
head/tape load mechanism.  If your humidity is higher than 50% and you
have a high dust level, you should be cleaning the drive at least once a
week.

There is absolutely NO WAY for the software device drivers in the Linux
kernel to cause the tape hardware to do anything that it's not supposed
to do.

At this point, the only thing that can be done is to clean the device in
a disassembled manner (meaning you can't do it).

Once you get the drive back, please invest in cleaning tapes to keep it
from happening again.
