Actually it has been fixed now, thanks to the feedback you sent, and merged into Greg's development kernel. Maybe it has found its way into some upstream kernel trees as well. A simple answer might be to remove the hci_usb_wait_for_urb() routine entirely once the UHCI change has fully percolated upward. While strictly speaking it should be there, problems during disconnect will be very rare.
