If it works initially, before you unplug the device, then interrupts are 
enabled.

I don't know enough about ACPI to comment on that, but if the same thing 
happens with ACPI support turned off...

Well, if loading it doesn't help this is a moot point.

This is strange, no question.  I'm out of ideas for the moment, and I 
haven't had a chance to go back and compare the initialization code with 
usb-uhci.
