Are you using a USB controller by VIA Technologies? If you are, this patch may help a little: http://marc.theaimsgroup.com/?l=linux-usb-devel&m=108516625726685&q=raw Your log shows the card reader responding in an illegal fashion, which kills VIA controllers. That would explain why you have to reboot.
