I have assisted someone who has the same modem on a mandrake 10 box.

When I say assisted I mean I attempted to get the beast running, but failed to 
get it to run reliably.

The 'make install' process failed to create a soft link between the character 
device and /dev/modem.

The modem failed to establish a reliable connection the computer owners isp, 
but connected to my isp with no problems.

The best way to use kppp's query modem feature, is by trial and error create 
soft links to /dev/modem from the following;

%dev (c, 212, 0) /dev/slamr0
%dev (c, 212, 1) /dev/slamr1
%dev (c, 212, 2) /dev/slamr2
%dev (c, 212, 3) /dev/slamr3

This  above info has been copied from the ./slmodem-2.9.9/scripts/slmodem.spec 
file.

The more adjustments tweaks etc we attempted to get the modem to work the 
deeper we seemed to dig ourselves into the mire.

I have not heard further from the computer owner. I fear that he has given up 
on Linux as too difficult.
