Code 16 is generated by pppd and means:

"16     The link was terminated by the modem hanging up."

How bloody helpful, we kinda knew that!

I have asked you twice before to get the modem init strings from the
windows install, and posted detailed instructions on how you might do
that. Please follow up on that as the next stage.
