Code 16 is generated by pppd and means: "16 The link was terminated by the modem hanging up." How bloody helpful, we kinda knew that! This page suggests that the modem init strings are important: http://www.linuxquestions.org/questions/showthread.php?t=61326 I have asked you twice before to get the modem init strings from the windows install, and posted detailed instructions on how you might do that.
