Do you need this with our without 3d acceleration?  There are mutliple
ways to do it but each has it's own pros and cons.  You might want to

You cannot make the laptop display be the main display when using the
external monitor with nvidia drivers.  they have hardcoded that into the
drivers.  You can arrange them left right top bottom orientations.
