Tell me.. Do you use the wacom kernel module from the linux kernel source? It 
has a 'defect' that is corrected in the code from the linuxwacom project 
which causes a prox-out to be sent as soon a you move the stylus outside the 
normal area and not after leaving the tablet completely (and at that time the 
X/Y coords are set to zero in the X-driver). For me that is a perfect fit 
with your remaining symtoms...
