The policy was a bit more... interesting than you describe.  In effect, 
there has been a hierarchy of practises, with the top of the hierarchy 
being an explicit statement by the appropriate "authority" associated with 
a particular ISO country code entry.

Practises "lower" on the hierarchy have been in place so that Internet 
operation did not need to wait for the relevant, associated authority to 
get into the decision path.  However, the ultimate test has been what IANA 
did with an existing delegation, given IANA receipt of a directive from an 
associated authority.  And, in fact, it has followed that 
directive.  (Apparent exceptions have had to do with determining whether an 
entity was, in fact, duly or sufficiently "authorized".)
