Any chance they got a new phone? I had a user who had this going on a few years ago and it turns out they got a new phone, gave their old phone to their kids, and thought they had wiped all the settings—but it was still set to sync contacts. After the next password change they started getting lockouts and I traced it back to her home IP, had her bring the phone in, and hard reset it. Problem solved.
