Places to look:
Radius logs if Radius used
IIS if it proxies authentications
Logs on the AD server that the lockout occurred on
VPN logs

On all devices clear any saved passwords:
Clear WiFi, E-mail and VPN saved passwords.
Do they use RDP?
Are there any other applications that authenticate that can save passwords?  
i.e.:  Lync / IM

User changed password.
Keeps getting locked out.
Removes email from phone.
Resets password.
Keeps getting locked out.
Logs or events show attempts from the exchange server.
What tool can I use to determine exactly what is causing the bad attempts that 
are locking her out?
