I'm on Debian Lenny + backports for LTSP. I just copied files yesterday
to a USB flash drive so I know basic localdev works.. Let me try my USB
DVD-ROM... (hold music)

Ok, so I'm seeing 2 things:

1) Plugging in my USB DVD-ROM without media shows icon on my desktop
"usbfloppy-sr0" (hard disk icon). Obviously there are no files when I
double-click it. When I insert a CD-ROM (data) disc, wait about 15
seconds for the drive to settle, and double-click again, still nothing
inside.

2) Plugging in my USB DVD-ROM *with* media already in it shows the same
icon *with* the files. I cannot eject the disc from drive (it's probably
locked when mounted right?) so this method works.

Not sure what that would mean for internal IDE/SATA/etc. CD-ROMS or if
they behave the same as USB (would you have to insert the media before
the terminal boots?)..Anyway..both are mounted as follows:
