Can you swap this drive into another machine ans see if the issue
follows the drive ?
Also, if you can swap another drive into the machine having the issue
and it works, then the rest of the machine should be ok, and it's more
likely to be software (including possibly drivers or the kernel).
Did you run fsck on the impacted drive ?

On the other hand, doesn't appear to be much evidence so far that the
drive is faulty.
