This is strange, as it works for most users (including me). As this is not a
very helpfull answer, I'll try to find some reasons why it won't work for you.

* Numbering/Labelling does not work for inline equations (Ctrl-M), 
  only for display equations (Alt-m d on my German LyX).

* Normally, (for a display equation on its own line) a blue (#) should

It does not matter where in the equation the cursor is (as long as it is in
the math-box.

When writing some label in the Insert>Label dialog, I get a blue
("my_label") in the right margin
