Could you cut down the UserGuide to a single equation that 'works', and create the same equation from scratch in a new doc, save both as .lyx and search for differences in the file? I am fairly sure we did nothing to 1.1.6 math that may cause such strage behaviour. In any case, you could try with 1.2.0preX just to make sure that similar problems do not creep into 1.2.0.
