If you move one app and place it on top of another app, a folder will be 
automatically created for you, and it will be given what it thinks is an 
appropriate name determined by the apps you are trying to put in to it.

The first thing to do then, is find an app you want to put in to a folder with 
another app. Double tap it, but after the second tap, do not lift your finger 
from the screen, but instead, slide your finger to where the second app is that 
you want to be in the same folder. Voice Over will tell you what is happening 
as you move your finger, so will announce when you come across the other app. 
When VO says "on [other app name" lift your finger from the screen, and this is 
when the folder will be created, and will have the two apps in it. By sliding 
your finger in the usual way, you will see the app names in the folder, and 
also see an edit field for the folder name, so you can change it if the default 
isn't of your liking. Most of us have to spend a bit of time getting use to 
moving apps around the screen, and sometimes, if you are not quick enough, and 
hold your finger on a second app for too long, a folder might get created that 
you didn't want, but don't panic if this happens,
  just locate the app you are trying to work with from within the wrongly 
created folder, and slide it out of that folder, and the folder will disappear, 
but not the apps you mistakenly put in it. If at any point you feel a bit lost 
or confused, just press the home key, and that will take you out of editing 
mode, and back to your home screen  -
