My bet is they are just very, very disorganised.
I suggest you *telephone* the editor and tell him/her the story. Get him/her to agree to a �100 fee - your original �50 plus compensation for hassle. �100 seems reasonable value, anyway, for use of the pic and the story. Ask for a deadline by which time you can expect your cheque.
Follow your phone discussion with a snail-mail letter.
If the cheque doesn't arrive by the agreed day, telephone again. And again.
The one possible problem is that you may find it hard to get the editor to take your call.....in which case you have to keep trying.
