It can be MacFUSE, sshfs, your server, or a combination.

I've heard this before in the context of sshfs so there's a higher
chance of it being some bug in MacFUSE and/or sshfs. There was in fact
a bug that I fixed a while ago that should have fixed this. After
that, I have not been able to reproduce it myself.

What would help is if you can come up with a reproducible test case--a
sequence of steps that will predictably cause this to happen. In any
case, please provide as much detail as you can about the circumstances
in which this happens.
