Hi Dan, I would go back to the basics.
1. Repair directory damage 2. Install all current updates 3. Repair disk permissions 4. Macjanitor (maybe) Still no luck? Try and create a temporary account, login to it and see if the problem persists. If so, you may want to perform an install and archive of the system.
