I have not programmed the OS X sound system but am guessing the bug is in there somewhere. Just a WAG that it gets submitted but not played immediately.

Here's a suggestion: I use zero spam-filtering software (other than the Mail rules). Do you guys who see the sound failure happen to be running any add-on packages like Spam Sieve or equivalent? Or any other add-on packages to Mail?
